*Please don't forget to follow the format (especially putting "Yearbook:" because it acts as a filter. Submissions not following the format will automatically be transferred to the trash bin.

If you are not able to submit your write-up by the deadline, a template will be used by the editors to construct a generic write-up for you, so please submit on time.

If there are any questions/clarifications, you may ask Isay Caguioa of Charm or Mia Pangilinan of Muon.

Fill up the form truthfully =o

Print your FULL NAME, including your middle name

Student No. refers to the one on your ID. :P

Please try to include all of your contact information such as your cellphone number and email address, as well as those of your parents.

Club affiliations and Leadership positions should be legitimate.

Awards received should likewise be legitimate. These would include international, national, interschool and approved PSHS competitions and events (ex: Math Intersection, KKKwiz). You may also include under 'achievements' if you qualified to be part of any teams (Team Bio, Team Econ, etc.).

For those people who still don’t have a permanent home address or telephone number (due to the typhoons, or other reasons), just indicate the information that you can’t provide and wait for further instructions from the data gatherers.

Batch Shirt

For Family Day, November 14, 2009, we will be accepting more orders for the batch shirt.

Just tell Cam Carag of your orders.

Sizes: XL for Kids, XS, S, M, L, XL, XXL, XXXL

It's 200 Php per shirt.

Place your orders on or before October 31, 2009. Late orders will not be placed.
